Your Ultimate Health Care Provider

ABOUT US
Foundations Hospice is founded in faith, service and compassion. Although we may be a new name to the industry, our team is not new to hospice. Together we have decades in the healthcare industry. Our local ownership means taking special care of our neighbors and friends in the St. Tammany, Tangipahoa and Western Livingston Parish areas. Contact us at any time for more information.
Hospice care is FREE
for people with a qualifying diagnosis.
What is Hospice?
Hospice care is health care focused on maintaining or improving your quality of life once you've been diagnosed with an illness, disease or condition that is considered terminal or unlikely to be cured.
Why Use Hospice?
The goal of hospice care is to ensure that you maintain your quality of life throughout pain management, symptom control, and various other therapies. Because of the highly detailed attention and supervision of care, people live longer when on hospice.
Who Can Use Hospice?
There are many people who could qualify for the extra support of hospice, but simply don’t know to ask about it. We are happy to provide a free consultation to help you determine if hospice care is available to you and work with your doctor, or ours, to obtain the needed recommendation.
Where Can You Receive Hospice Care?
Hospice care can be provided in your own home, a long-term care facility such as assisted living, or the hospital. The Foundations Hospice team is here to support you, and that means helping you wherever you are most comfortable.
