Why Trust Foundations Hospice?

The Local Difference: Decades of Experience, a Servant Heart

Although Foundations Hospice may be a new name, our team is not new to hospice. Together, we bring decades of combined experience in the healthcare industry, and every member is carefully selected for their expertise and commitment to providing exceptional care.

Our local ownership means we take special care of our friends and neighbors across parishes including Ascension, Jefferson, Orleans, St. Tammany, and East and West Baton Rouge. We provide 24/7 support for patients and their families.

Care That Is Personal, Local, and Led With Integrity

Choosing hospice care is not only about clinical support. It is about who is showing up for your family and how present they are during one of life’s most meaningful seasons.

Foundations Hospice was built on the belief that end-of-life care should be personal, accountable, and deeply connected to the community it serves.

Locally owned and operated in Louisiana,

allowing care decisions to be made close to home, not in a corporate office

Direct owner involvement,

including meeting with every family at the start of care to ensure trust, understanding, and alignment

Expanded comfort-focused support,

including palliative care services as programs and licensing allow, designed to help manage symptoms and improve quality of life

A relationship-centered approach,

where families are known by name and care plans reflect individual values, not templates

This level of involvement allows our team to remain present, responsive, and committed to honoring each patient with dignity.

Services Designed to Support the Whole Family

Hospice care supports more than physical symptoms. It supports emotional, spiritual, and practical needs for both patients and those who love them.

Palliative Care

Licensed palliative care services may be available depending on program approval and eligibility, providing additional symptom relief and support earlier in serious illness.

24-Hour On-Call Support

A nurse is available day and night to answer questions, manage symptoms, and provide reassurance when concerns arise.

Medicare, Medicaid, and Select Private Insurance Covered Hospice Services

We provide hospice care that is covered by Medicare and Medicaid, and is also covered by some private insurance plans. This coverage includes medications related to the hospice diagnosis, medical equipment, supplies, and interdisciplinary care.

If you are unsure about eligibility or timing, our team will explain hospice benefits in clear, simple language and help you understand what is covered and what to expect.

When Is the Right Time for Hospice?

Many families wish they had called sooner. Hospice care can begin when a physician determines that comfort-focused care is appropriate, often alongside ongoing support from loved ones.

If you are asking questions, it may already be the right time to talk.

You do not need to have everything figured out before calling.
