Palliative Care

Palliative care provides specialized support for people living with serious illness. Its purpose is to improve comfort and quality of life by helping manage symptoms, reduce stress, and coordinate care. Palliative care can be provided alongside treatments meant to slow or treat the illness, and it is appropriate at any stage of a serious diagnosis.

Palliative care also supports the people caring for the patient by offering guidance, education, and practical help during a time that can feel medically complex.

What Palliative Care is Designed For

Palliative care is designed to add an extra layer of support when symptoms or treatment side effects are affecting daily life.

This care is often used to:

  • Improve comfort by managing symptoms such as pain, shortness of breath, nausea, fatigue, appetite changes, and sleep issues
  • Reduce distress related to illness, including anxiety, fear, and emotional exhaustion
  • Support decision-making when there are multiple treatment choices or care settings
  • Coordinate care across providers to reduce confusion and prevent gaps in care
  • Help patients stay as active and comfortable as possible, for as long as possible

This stage of care focuses on relief and support, not a prognosis. It is based on needs, not a timeline.

Who Palliative Care Helps

Palliative care is designed to add an extra layer of support when symptoms or treatment side effects are affecting daily life.
This care is often used to:

  • Cancer
  • Heart disease and advanced heart failure
  • Chronic lung disease (COPD and other pulmonary conditions)
  • Kidney or liver disease
  • Neurological conditions such as Parkinson’s disease, ALS, or dementia
  • Serious infections or complex medical conditions causing ongoing decline

If symptoms, side effects, or stress are increasing, palliative care may be appropriate even if treatment is continuing.

What to Expect From Our Palliative Care Services

Palliative care is coordinated and individualized. Services vary based on needs, but may include:

  • Symptom management and comfort planning in coordination with the treating physician
  • Medication education and help addressing side effects or treatment burdens
  • Emotional and spiritual support based on patient preferences
  • Help with advance care planning, including identifying goals, values, and priorities
  • Care coordination and communication support across specialists and settings

Palliative care is not one-size-fits-all. The plan is built around what matters most to the patient.

How Our Palliative Care Supports Families and Caregivers

Serious illness affects the entire household. Palliative care can help families feel more informed and less overwhelmed by providing:

  • Practical education on symptoms and what to watch for
  • Help preparing for changes in condition over time
  • Support navigating difficult conversations and decisions
  • Guidance connecting to community resources when needed
  • Families do not have to manage these concerns alone. Palliative care helps create a clearer path forward.

Palliative Care vs Hospice Care

Palliative care and hospice care both focus on comfort, but they are not the same.

Palliative care:

  • Can begin at any stage of serious illness
  • May be provided while curative or disease-directed treatments continue
  • Is based on symptom burden and support needs

Hospice care is appropriate when a physician determines a patient is eligible and the focus shifts fully to comfort rather than cure. For some people, palliative care later becomes hospice care. For others, it remains supportive care while treatment continues.

When to Ask About Palliative Care

Palliative care may be a good fit if:

  • Symptoms such as pain, nausea, breathlessness, or fatigue are limiting daily life.
  • Treatment side effects are difficult to manage or causing repeated setbacks.
  • Hospitalizations or ER visits are becoming more frequent.
  • You feel uncertain about next steps and want clearer guidance and coordination.
  • Caregiving demands are increasing and support is needed at home or in a facility.

Starting the conversation early often leads to better comfort and more control over care decisions.

Palliative Care Setting

Palliative care may be provided in the setting that fits the patient’s needs, which can include:

  • Home
  • Assisted living communities
  • Nursing facilities

The goal is to support comfort and coordination without disrupting ongoing treatment plans when possible.

Coverage and Referrals

Palliative care coverage depends on the care setting, the services provided, and insurance plan details. Services may be covered by Medicare, Medicaid, or private insurance in many situations.

Our team can explain how referrals work and what services may be available, so families understand options before making decisions.

Where do I even begin?!? I was given 24 hours to make a decision once my mother was being early released from a skilled nursing facility. In a panic, I started googling. It was after hours, around 5:30 pm. I left a message & the owner called me back within like 90 seconds! & our entire experience thereafter has been very similar! He worked with me until almost 10 pm that night, then met with my mother & I the next day where he spent about 2-3 hours answering all of our questions, he arranged transport for her & had her equipment delivered within an hour of her arriving home! Mr Mark is an incredible soul & he does this work with such conviction & passion. He may own the company, but he is very much the boots on the ground & he is sincere & loves helping others. His energy is infectious & his heart is pure. If you say you need anything at all, they are there in a flash. Call day or night & they are there to support you. They are not only there to help care for your loved one, they are there for YOU the caretaker, just as much as the patient! They sometimes know what you need before you do! After a particularly rough morning, our nurse could tell that I was overwhelmed & feeling very defeated. She told me someone will be calling & coming by soon. She had asked the social worker to come to my house to check on ME! When I welcomed her in to meet my mother, (the patient), she said, “I will meet her. But I’m here for YOU.” We then sat down & she allowed me to express all the things I was feeling. She helped guide me through my emotions & validated my feelings. She helped me see my role more clearly & gave me the reassurance I so desperately needed. That little “therapy session” completely turned my day around & was the boost I needed to get through the day. I’m a yapper, so I could go on & on for hours about how much this company has impacted my life & my family. Caring for a loved one on hospice is hands down the hardest thing one can ever endure in my opinion. Foundation’s has been a guiding light for us, they’ve been my biggest supporter & they are always seeking ways they can help us further. To say they go above & beyond is an understatement. I don’t know much about other hospice services, this is my first & only experience with this. But I do believe God himself put Foundation’s in my path. You won’t find another service who cares as much as they do!

My mother was being bathed daily in her last months of life by Koko. Koko was so caring and gentle with her. I’ll hold that as a dear memory because it was a comfort to observe her manner. She kept maintaining Mom’s humanity and dignity as a primary task.

Momma has been with Foundations for about four months now, give or take. These folks are amazing. First off, know that hospice is not only for very end of life, I didn’t know that, they help in many other ways too. After a nasty fall and surgery and now wheelchair bound among other things she has, we decided to reach out to Mark of foundations (owner) for help and advice for momma. Love us some Mark!!!! Besides where momma lives, next best decision we made!!! So much weight off our chests knowing we have an extra set of eyes helping us take care of momma and where we can no longer easily take her to doctors and nurses, etc., they come to her. Mark and his staff, starting with Lindsey (one of his territory managers), their nurses (momma’s being Rachel and Mackena), mommas care person who comes to shower her, Tiffany, and so many more staff she sees, they are all blessings and God sends! I cannot speak highly enough of this company. Unsure? Call them they will come out and help with your decision. If I could have given above five stars I would have!!!

Foundations Hospice has an excellent and caring team who cares about your loved one, but also you. Ashley, the nurse who i could call anytime, took care of my husband , who has passed and my Dad, who is in the Nursing home. My anniversary was coming up, and she knew my husband always got me something. She brought flowers and took a picture of us. He passed 3 days later. She framed that picture and brought it to me at his funeral. I cannot tell you how she got me through the funeral, Don, the Chaplain knew when I needed someone. He was there to listen and pray with me. Sheika, the aide, was excellent. I was amazed to see her move a grown man and how she cared for him. Brad, the social worker was great. If I missed someone, it wasn't intentional. I am telling everyone about my experience with Foundations Hospice. They are the #1 place to use. You won't be disappointed.
